What technique does Dr. Stevens use for breast augmentation surgery?

  • Adits, small holes in the skin that are made with round, skin-biopsy punches, are created at the sight of the surgery.
  • These adits are only 1.5 mm long. They are the access sites in which a cannula, the tool used for liposuction, is passed in and out during the liposuction process.
  • The cannula Dr. Stevens uses is quite small. He custom-designed it for his organic surgeries.
  • A large volume of very dilute lidocaine (local anesthetic) and epinephrine (capillary constrictor) is injected into the fat below the skin, also called subcutaneous fat.

  • The fat used for the lipofilling is usually taken from the abdomen, flanks, waist and/or thighs.
  • The "micro" cannula is inserted into subcutaneous fat in these areas through the adits.
  • The fat is removed in a very efficient, gradual and controlled manner so as not to "bruise" the fat cells.

  • Adits are so small that it is not necessary to close them with sutures. They promote copious postoperative drainage, which in turn reduces post-operative bruising, tenderness and swelling.
  • Such tiny holes usually disappear without scars within a few weeks after liposuction.
  • The fat tissue accumulated during the liposuction is put into several sterile tubes.

Using centrifugal force (fast spinning to seperate parts of a substance), the tubes containing the fat tissue is separated into three parts:

  • oil from broken fat cells (not suitable for filling);
  • blood/anesthesia fluids (not suitable for filling); and
  • living fat cells (about 20% of the total) which is only part used for liposuction.

  • The living fat cells are treated with great care and precision for use in the surgery.

With Dr. Stevens' surgical technique:

  • the purified fat is inserted through puncture holes of only 1mm at the areas predetermined by Dr. Stevens.  Naturally, these areas will be different for each person.
  • the fat is inserted drop-by-drop and evenly distributed -  no blobs of 'mixed parts' as in the past!
  • blood vessels can supply all living fat cells.
  • more than 90% of the inserted fat cells remain alive.  Permanently!

This new technique that Dr. Stevens uses is far superior compared to the techniques used in the past,  the results of which were often disappointing and impermanent because:

  • fat cells were unnecessarily bruised during extraction.
  • anesthetic fluid, blood and oil, which made up 70% to 80% of the extracted fat, were used for lipofilling, which hindered permanent results.
  • when fat is transplanted to another site, it needs new blood vessels to grow into it to provide nourishment for the fat cells.  
  • a large and thick needle was used, which could insert only "blobs" of the mixed fat tissue, resulting in a lack of refined control over the lipofilling.

FAQ

Recovery Time
Pain and discomfort can last for 2 to 3 weeks, especially in the area where liposuction was performed. Immediately after the surgery, the breasts will be quite swollen and the areas where liposuction was performed will feel bruised. After 4 to 6 weeks approximately 80% of the bruising and swelling will have disappeared. After 3 months you can get a good impression of what the final result will be.

Post-Operative Care
Within a few days after surgery, you will be able to perform your usual activities. Social activities should be reduced for one week after surgery if they involve the areas involved in the surgery such as thighs, hips, tummy and the breasts. Long walks, straining and athletic activities should be completely eliminated for one month.

Possible Complications
Complications related to the procedure are rare and, when they do occur, they might be infection, hematoma, asymmetry and/or an irregular skin surface.

These complications are rare and can be easily corrected without problems. Although this is a low-complication risk procedure, antibiotics are prescribed to prevent or cure any possible infection. Possible irregularities or asymmetry can be easily treated with secondary outpatient surgery with local anesthesia or general anesthesia six months, if prefered, six months after the first lipofilling procedure.
